Grace Group Home is an assisted living facility located in Minneapolis MN.

Our staff has experience in providing vulnerable adults with a wide range of cognitive challenges or developmental disabilities, acquired traumatic brain injuries, chemical dependency and/or mental health diagnoses with the support they need to establish a healthy and safe life.
We aim to include the individual’s support system when curating care plans because we understand that a collaborative partnership between family, and care teams (i.e. providers, case managers etc.) aids in improving the health and quality of life of that resident.

Our services
Customized Living Services
This person centered Assisted Living facility provids 24 Hr Customized Living and housing Services to 18+ clients . Highly trained direct support professionals provide assistance to our residents with:
- Daily living activities
- Medication administration,
- Mental health,
- Cognitive or behavioral concerns,
- Socialization and increasing community engagement.
24-Hour Assistance
Onsite staff is available round the clock to provid assistance to residents.
- Awake Night Supervision
- Weekly Nurse visit and on-call 24-hour medical assistance
- Coordinate transporation requests as needed
- 3 home cooked meals daily
Each bedroom is also equipped with wireless emergency call pager system that alerts staff when needed.
Accommodations
Who do we accommodate: All individulas on a Waiver program or recieve other funding for assisted living facility (ie.Private Pay, Medical Assistance)
